Below you will find the Paperwork needed to Join Pack 131
To register for Pack 131 each Scout needs to complete a Scout Application, 2 copies of a Medical form & the Pack Code of Conduct Acknowledgement. Parents attending Cub Scout events will need a Medical Form and Youth Protection Training Completion Certificate on file with the Pack. Once completed please return to the Pack. The Medical form is now editable so you can type in your information and simply print 2 copies!
