Adult Required Forms

Adults participating in certain Pack Activities need to have completed BSA Youth Protection Training, Code of Conduct Acknowledgement and a BSA Medical Form.

To take Youth Protection training click the link below.

Look for the directions in the section entitled "New to Scouting? Click here to take Youth Protection Training.  

​You will be directed to the My.Scouting.org portal and prompted to create an account.  Its not a bad idea to record your username and password.

Once in the MyScouting.org portal, click on E-Learning and take the Youth Protection training.

Upon completion, you should print the a certificate of completion to submit to the Pack. A Pdf of this certificate can be uploaded through the Pack Website.  

Youth Protection Training expires after 2 years and will need to be redone bi-annually.
BSA Youth Protection

BSA Medical Forms

Boy Scouts of America requires a current Medical form to be on file for each person participating in Pack Activities.  This includes both Cub Scouts and their participating parent.  Cub Scout forms need to be submitted upon request and should be delivered to either their Den Leader or the Cubmaster.

Adult forms can be placed in a sealed envelope, and delivered to the Den Leader or Cubmaster.  Please write  the adult's name and the Cub Scout's name on the envelope as well as the date the form was completed.

Pack 131 Code of Conduct Acknowledgement

The Pack Committee requires each family participating in any Cub Scout event to have completed a Code of Conduct Acknowledgement.  The form is attached below.
